The ACORD 25 is the certificate of liability insurance that landlords, vendors, and general contractors ask for before they let you on site. It shows coverage that already exists. It does not change the policy. Most agencies retype the same named insured, carriers, and policy numbers on every certificate, and one wrong digit sends the request back. The producer or account manager fills the ACORD 25. The certificate holder is named in the bottom block, and the producer signs the certificate. The insured does not sign it.

What ACORD 25 asks for
The top of the form carries the producer, the insured, and the insurers with their NAIC numbers. The coverage grid lists each policy type with its policy number, effective and expiration dates, and limits. The bottom blocks hold the certificate holder, the description of operations, and the cancellation notice language.
Why the description block causes most rejections
Additional insured and waiver of subrogation status is written in the description block, usually with endorsement form numbers such as CG 20 10 or CG 24 04. Holders check those numbers against the policy. A profile stores the numbers once, so the description matches the policy every time.
What the certificate does not do
An ACORD 25 confirms coverage that exists on the date it is issued. It does not extend coverage, and it does not replace an endorsement. If a holder asks for coverage the policy lacks, the broker needs to add the endorsement first.
